8. TROPAMIC: The role of troponin kit in the community for diagnosis of acute myocardial infarction

Abstract:
Troponin T kit (TK) testing has been established as a simple, quick and reliable test for the assessment of myocardial necrosis and overall prognosis in patients presenting with chest pain to the emergency room. The use of TK in the community clinic setting has never been investigated. The goal of this study is to assess the utility of TK testing in patients presenting with complains of chest pain to their family physicians in the community.
Patients are recruited at 32 community clinics in Jerusalem. Patients with at least 20 min of chest pain, beginning at least 8 hours prior to presentation are included in the study. Subjects initially undergo clinical assessment (CA) by the family physician who decides whether emergency room (ER) referral is indicated. Following this decision, TK testing is performed. Patients with negative TK and negative CA are sent home and all others referred to the ER. CA and TK testing are then compared to the final diagnosis. Telephone evaluation is performed after 45days to establish short-term outcome among all the patients.
