ד"ר אפרמיאן מנהל את שרות המרפאה למחלות בלוטות רוק בבית החולים הדסה, המטפל בחולים הסובלים מבעיות של יובש פה וריור יתר מכל רחבי הארץ. מרפאה זו ייחודית בתחומה בארץ ומאובחנים ומטופלים בה חולים בעזרת אמצעים ופרוטוקולים חדשניים.
ד"ר אפרמיאן סיים את לימודיו בבית הספר לרפואת שיניים של האוניברסיטה העברית והדסה בירושלים והמשיך להתמחות במחלקה לרפואת הפה של בית החולים האוניברסיטאי הדסה עין-כרם.
במקביל ד"ר אפרמיאן סיים תואר שני בוירולוגיה מולקולרית ותואר שלישי בביולוגיה תאית באוניברסיטה העברית.
לאחר קבלת תואר המומחיות ברפואת הפה בחר ד"ר אפרמיאן לעסוק בתחום בלוטות הרוק ושימש כעמית מחקר במשך שנתיים במרכז המוביל תחום זה בעולם במכון הבריאות הלאומי של ארה"ב.
שם גם החל לפתח בלוטת רוק מלאכותית לשחזור בלוטות רוק שנהרסו בחולי סרטן ראש צוואר מוקרנים.
עם סיום שהותו חזר ד"ר אפרמיאן למחלקה לרפואת הפה ותוך זמן קצר התמנה לאחראי מרפאת מחלות בלוטות הרוק ותחת שרביטו עברה המרפאה שינוי ניכר, הן בתחום הטיפול בחולים והן בתחום המחקר.
ד"ר אפרמיאן עוסק במחקר בסיסי וקליני ופרסם מאמרים רבים בתחום.
מחקריו של ד"ר אפרמיאן זיכו אותו בפרסים ומלגות יוקרתיות כמו פרס פולקס, פרס בת-שבע דה רוטשילד היוקרתי לרופא חוקר לשנת 2004, מענקי מחקר של האקדמיה הלאומית למדעים, משרד הבריאות והאגודה למלחמה בסרטן
בין שאר עיסוקיו ד"ר אפרמיאן הוא אחראי תכנית ההתמחות במחלקה לרפואת הפה ומשמש כיו"ר הועדה לעבודות מחקר לתואר ד"ר בפקולטה לרפואת שיניים.
