Since joining Hadassah in 2000 I have been a senior physician in the Department of Geriatrics and Rehabilitation. My duties included the medical responsibility for different wards, including Geriatric Rehabilitation, Sub-acute care, Hospice, Home Hospice, Long- term complicated care, and the Geriatric Emergency Room. In addition to the medical management of these wards, the administrative duties have included the management and orchestration of multidisciplinary teams, demanding close cooperation of nursing staff, physiotherapists, occupational and speech therapists, and social workers.

Experience in Community Geriatric Administration

 

On completing my specialization in Geriatric Medicine in 1998, I became consultant geriatrician in a Comprehensive Geriatric Assessment clinic, and subsequently became the Medical Director of the Jerusalem Home Hospital and Home-Care Unit, both of which are divisions of the Institute for Geriatric Medicine, Clalit Health Services, Jerusalem District. The Home Hospital is an innovative and unique service providing intensive comprehensive medical care at home to 300 patients simultaneously, as an alternative to in-patient medical, geriatric, or hospice care. Management of this model of community-based health care has required the planning, coordination and administration of a large multidisciplinary team of health professionals, utilization and maintenance of considerable logistic backup, as well as financial accountability to the Jerusalem Directorate within the Clalit Health Services.

 

During the years of 2000-2004 I served as the Acting Regional Geriatrician and the Director of the Institute of Geriatric Medicine.

 

Since 2006 I have been a member of several National Advisory Committees at the Clalit Health Service Central Office. Working topics and areas of interest have included the definition, implementation and follow-up several areas of Geriatric care

Scientific Biography

 

Since 2003 I have become increasingly interested and involved in the research efforts associated with the Jerusalem Longitudinal Study. Established under the auspices of Professor Jochanan Stessman in 1990, the study has successfully followed up a cohort of Jerusalem residents born 1920-1921, with comprehensive assessment of subjects being performed at ages 70 (1990),  78 (1998) and again at age 85 (2005). A wealth of data exist, allowing both cross-sectional and longitudinal analysis, and few such studies have successfully spanned a community based cohort of this age group over such an extended time period, and the study is unique within Israel and the Middle East.

 

The study’s aims are broad, and briefly stated include the description and creation of a profile of the ageing process among an age homogenous cohort; an identification and clarification of the impact of social, economic, cultural, behavioral, and ethnic factors on patterns of ageing; an understanding of the importance of health related behaviors on successful ageing; a delineation of the relative significance of premorbid conditions, chronic illness and various treatments on subsequent health and functional status, morbidity, and survival.

 

My personal contribution to the conclusions that are being drawn from the study has focused on understanding the impact of several common chronic complaints on the ageing cohort. Impaired vision, chronic back pain, poor sleep satisfaction, nocturia, and chronic anemia, as well as distant trauma of Holocaust exposure, have each been separately studied and significant long-term effects and predisposing factors have been clarified. More recently my attention has shifted to the possible health and survival benefits of various common behaviors, such as leaving the house on a frequent basis, reading habits, social and cultural activity. In particular we demonstrated the importance of remaining physically active and its influence on maintaining functional independence, well being, and improved longevity.

 

Current directions include the investigation of the development and patterns of dementia and depression from ages 70- 85; the role of cardiac morphology and function in successful aging and its contribution to disease, functional status, and cognition among the very old; determinants of health care utilization among the oldest old; as well as a comprehensive description of the trends in overall health and psychosocial status among the cohort throughout follow-up.

 

An additional area of my research has been the evaluation of the impact of the Jerusalem Home Hospital on the health service utilization among the older population. Various cost-benefit analyses have reproduced the financial benefits and economic justification of this novel form of health care provision. Future research is being aimed at evaluating feasibility and potential benefits of specific modes of at home among various patient categories, including long term continuous mechanical ventilation at home, and chronic heart failure.

 

Finally the area of palliative care remains an area of potential research. My experience in both Home care and In-patient care of dying patients has provided the opportunity to investigate area of palliative sedation of terminal patients at home, and the impact on survival of high dose opiates among Hospice patients.
