§         Payment voucher from the health fund

§         Referral from the health fund doctor (if any)

§        Identity card or any other identification document

§        Maccabi Health Fund members must bring their magnetic card.

§        Patients who arrive without a voucher will be asked to pay for medical treatment at the reception desk.

§        All usual forms of payments are accepted: credit card, cash, check or foreign currency.

§         Patients who pay for treatment, and at a later stage produce the appropriate payment voucher for the treatment carried out, will receive a full reimbursement from Hadassah.
