Payment voucher from the health fund
Referral from the health fund doctor (if any)
Identity card or any other identification document
Maccabi Health Fund members must bring their magnetic card.
Patients who arrive without a voucher will be asked to pay for treatment at the reception desk.
All usual forms of payments are accepted: credit card, cash, checks or foreign currency.
Patients who pay for treatment, and at a later stage produce the appropriate payment voucher for the treatment carried out, will receive a full reimbursement from Hadassah.
