Physicians of the department provide consultations for all departments of the two Hadassah-University hospitals. They advise on facilitation of early discharge and on mobilizing appropriate community services for the patient at home. They recommend and make referrals to appropriate long term care facilities or to sub-acute geriatric medicine departments and they provide the direct link to the Home Hospitalization program of the Clalit Sick Fund.
